Any changes to an existing contract (i.e. address, phone number, new boat, new partner, etc.) must be submitted in writing via a Change Request Form. A $30.00 charge will be applied when adding a partner or changing a boat. There is no charge for address or boat name changes.

You may fill out a Change Request Form on-line and submit it electronically, or fax it to us at (312) 747-6598. If you wish to send us a hard copy, please mail it to:
